Checking In

You must arrive fifteen (15) minutes prior to your scheduled time for BOTH the written examination and for the skills evaluation. Please plan to spend the entire day. If you are late for the examination, you will not be allowed to test, and your fees will not be refunded. 

You will be required to check in for both the written examination and for the skills evaluation. You will be required to present proper identification.

What to Bring

You must have the following items with you when you take the examination:

  • Original Social Security card (cannot be laminated)
  • Valid current government-issued legal photo identification card that includes your signature
  • Application for Nurse Assistant Certification (HS 283B) with original signature from the RN responsible for your training that is dated after you have completed training or original California Department of Public Health (CDPH) Approval Letter with embossed stamp from CDPH 
  • Watch with a second hand
  • Non-skid footwear. Dress professionally, uniform preferred.
  • Two (2) No. 2 pencils (sharpened)
  • Eraser
  • Optional Item: Translation Dictionary (Translations only, no definitions)

No other materials will be allowed. You MUST have the above items with you on the day of testing. If you do not bring the above items with you on the day of testing, you will NOT be allowed to test, and your examination fee will NOT be returned.

You may bring a paper English translation dictionary which will be checked for notations and any other security issues. No electronic devices of any kind are permitted.

ID Card Requirements

You are required to bring your original Social Security card and one form of legal photo identification that includes your signature when reporting to take the examination. This is a mandatory requirement issued by the California Department of Public Health (effective January 2002). Photocopies of identification will NOT be accepted.

Examples of proper identification include:

  • Driver’s license
  • DMV Identification card
  • Passport or Passport card
  • Permanent Resident Visa/Alien Registration card
  • U.S. Military ID card

The first and last names or suffix (Jr., II, III), on the current government photo identification, social security card, Nurse Assistant Certification (HS 283b), or CDPH Approval letter must be the same name used on the registration application to register for the examination.

If you do not bring proper identification, or if your names do not match on all presented documents, you will not be allowed to test, and your examination fee will not be refunded. All documents presented at test site must be current, original, legible, and unaltered.

Security and Cheating

If you give help to or receive help from anyone during the examination, the examination will be stopped. The incident will be reported to the California Department of Public Health for review, and your examination will be scored as an automatic failure.

Consequently, any distribution of the examination content or materials through any form of reproduction, or through oral or written communication, is strictly prohibited and punishable by law.

Anyone who removes or tries to remove examination materials or information from the test site will be prosecuted.
